ISAR (Identification of Seniors At Risk) is a 6-item screening tool for seniors in the emergency department.
The ISAR screening tool, developed to identify patients at increased risk of functional decline, can also identify patients who are more likely to return to the ED. OBJECTIVES: To develop a self‐report screening tool to identify older people in the emergency department (ED) of a hospital at increased risk of adverse health outcomes, including: death, admission to a nursing home or long‐term hospitalization, or a clinically significant decrease in functional status. Detection of older people at increased risk of adverse health outcomes after an emergency visit: the ISAR screening tool. 2009 Feb;21(1):69-75. doi: 10.1007/BF03324901.Carpenter CR, Shelton E, Fowler S, Suffoletto B, Platts-Mills TF, Rothman RE, Hogan TM.Acad Emerg Med. The proposed screening tool (ISAR) comprises six self-report questions on functional dependence (premorbid and acute change), recent hospitalization, impaired memory and vision, and polymedication. The tool quickly identifies seniors who are at increased risk of a variety of adverse outcomes (including functional decline, death, institutionalization, return visits, high rates of hospitalization and use of other services).

Background: older adults are frequent users of emergency services and demonstrate high rates of adverse outcomes following emergency care. Hij kan worden aangevuld met de informatie die men van de patiënt of een begeleider verkrijgt . This paper describes the implementation of the tool in the Mount Sinai Hospital emergency department using a Plan-Do-Study-Act model; and demonstrates whether the tool predicts adverse outcomes. ISAR is gemakkelijk te gebruiken, het is een kort en snel instrument. Published by Oxford University Press on behalf of the British Geriatrics Society.

The Identification of Seniors at Risk (ISAR) instrument is one of the most commonly used and best-validated screening tools. The tool quickly identifies seniors who are at increased risk of a variety of adverse outcomes (including functional decline, death, institutionalization, return visits, high rates of hospitalization and use of other services). Background: tools are required to identify high-risk older people in acute emergency settings so that appropriate services can be directed towards them.
